Custom Provision Order Form (Adobe Acrobat PDF Form) - The Form is about 18 Pages. Print out the form, fill it in, then FAX it to us at 1-284-494-1121. (The items on this form are identical to those on the Automated Spread Sheet)

You will need the Adobe FREE Acrobat Reader to Open the Form. Note that the Form is NOT EDITABLE. You will need to Print the Form, Complete it and then FAX it to us.

If you DO NOT want to FAX all 18 Pages and Your Order is Short Enough, you may List your Order on the Short Order Form (page #18) and Fax it to us.

If you want to E-MAIL your Order using this Form you will need access to a SCANNER - SCAN your COMPLETED Form into your Computer. Save the Scanned Form as a PDF Document, or a JPG picture Document, or a Microsoft Image Writer Document and email the SAVED Document to us as an ATTACHMENT.

Alternately you may TYPE your Order into the BODY of a REGULAR E-MAIL (or a Word Document or Regular Excel Document) and E-Mail it to us. Make sure to include the Quantity, Item Code and a Brief Description.

We try to keep our on-line Static Price Lists up to date. Prices are subject to change without notice. All orders are processed at our Computerized Scanning POS System at the time of delivery and the final amount rung up then is what is finally billed to you. Apart from the usual volatility in Fresh Produce, prices are pretty stable.
